Missouri Licensed Care Manager

A company is looking for a Care Manager, LPN.

Key Responsibilities

Deliver telephonic care management and develop patient-centered care plans

Support quality gap closure and coordinate patient visits with providers

Communicate patient health updates and treatment plans to caregivers and providers

Required Qualifications

Active, unrestricted compact LPN license

2+ years of nursing experience in various care settings

Knowledge of pathophysiology and treatment protocols for common health diagnoses

Ability to analyze patient records and work independently in a remote environment

Proficient computer skills and experience with clinical data usage
