Licensed Practical Nurse
The Licensed Practical Nurse has the responsibility and accountability for assuring delivery of quality nursing care for hospice and home care patients under the direction of a Registered Nurse in accordance with individual patient needs, physician orders, home care and departmental policies. The LPN will provide hospice / palliative and home care services with respect for human dignity and the uniqueness of the patient, unrestricted by consideration of socio-economic status, personal attributes or the nature of the health problems. Maintains confidentiality of information regarding patient / caregiver, and all health care team members.
Education : Must be a graduate of a school of nursing. Must complete hospital general orientation and department orientation (Home Health & Hospice)
Licensure : Must possess a current and valid license as a Licensed Practical Nurse from the Indiana State Board of Nursing. Must have valid state driver's license, reliable transportation, and proof of auto insurance. Must have current CPR certification.
Other Qualifications : Must have two years of experience in home care, oncology, critical, med / surg or nursing home. Must be able to lift 50 pounds and assist with 100 pounds.
