The Licensed Vocational Nurse is responsible for delivering high-quality direct patient care under the guidance of a registered nurse. This role includes adhering to the care plan, implementing treatments, and working with a team to achieve positive health outcomes.
Key Responsibilities :
- Deliver direct patient care according to the Nurse Practice Act and company policies.
- Implement care plans developed by registered nurses.
- Maintain accurate and timely documentation aligned with care plans.
- Educate patients and families regarding diagnoses and care plans.
- Collaborate with healthcare team members and refer needs to appropriate disciplines.
- Follow all infection control measures and safety protocols.
- Utilize supplies and equipment efficiently.
- Engage in personal and professional development activities.
- Meet documentation standards and adhere to operational procedures.
- Perform other nursing duties as required.
Communication Duties :
Prepare clinical notes and update the primary physician as necessary.Report patient changes to physicians and coordinate necessary services.Collaborate with the Clinical Team Manager and others regarding patient care scheduling.Additional Responsibilities :
Participate in on-call duties as part of a rotating schedule.Demonstrate knowledge of Patient Rights and Privacy Practices.Ensure availability of necessary equipment and services.Maintain a patient case load and adhere to documentation standards.Work flexible hours to accommodate patient needs.Exhibit strong attendance and punctuality.Uphold confidentiality concerning all patient and agency information.Foster a positive agency image through appropriate professional attire.This role is vital to our mission at WellMed, which focuses on providing exceptional care to seniors. Required Qualifications :
High school diploma or equivalent.Graduate of an accredited practical nurse or vocational nursing program.Unrestricted license as a practical nurse in Texas.Current CPR-BLS certification or ability to obtain before employment.Minimum 2 years of clinical experience, ideally in home health or medical / surgical settings.Strong knowledge of nursing practices and regulatory standards.Able to perform necessary physical tasks and maintain visual and auditory acuity for nursing duties.Valid Texas driver's license and personal auto insurance as per requirements.Must complete 6-8 weeks of in-office training.Preferred Qualifications :
Experience with electronic medical record systems.Effective prioritization and clear communication skills.Ability to collaborate with multidisciplinary teams.Excellent observation, communication, problem-solving, and organizational skills.Physical & Mental Requirements :
Capacity to lift up to 50 pounds and assist with patient transfers.Ability to push / pull heavy objects as needed.Capability to sit / stand for extended periods.Competence in using office equipment and driving personal / company vehicles.Ability to receive and comprehend verbal and written instructions.Logical reasoning for complex situations.Work Environment :
This position involves working in various home environments, where exposure to a range of conditions (including potential irritants and infectious materials) may occur. The ability to manage stress and maintain composure is crucial in this role.
