Assignment

This is a 52 week contract working with the Bob Stump VA Medical Center in Northern Arizona to help provide care to Veterans in critical need. There is potential to extend each year for up to 5 years in total. Typical shifts will be 8 hours each day with a general Monday - Friday, 8- 5 schedule.

• Kayenta VA Clinic
• Page VA Clinic
• Polacca / Hopi VA Clinic
• Tuba City VA Clinic

Position Summary

As a Mental Health Nurse Practitioner , you will provide prescribed medical treatment and personal care services to patients with diseases and injuries seeking treatment in the clinic or medical center, as directed by physician or mid-level provider. You will collaborate with the core PACT Team (Primary Care Provider, RN and Medical Assistant) and expanded PACT Team including family / caregiver, VA, and community-based services involved in developing the patient care plan.
