Who You Are : You are a Certified Home Health Aide in California who is interested in working with an interdisciplinary team of health professionals helping individuals that have transitioned from a hospital to home care setting and assisting them to remain in the communityYou enjoy helping clients achieve their personal goals of remaining in the community.You want to work with a team that inspires professional growth and share our priority of providing quality case management and home health care to the community.

Note

• This is not a position for Caregivers, or Registry Aides. We are accepting applications from Certified Home Health Aides who have completed training as a Home Health Aide and received the necessary license issued from the State of California.

Service Region / Location : This is not a remote role and requires you to work out of the Chico office seeing patients in Chico and Butte County and in surrounding counties.

Schedule : Part-time work - Ranges from 2-29 hours per week, hours may vary.

Travel : You will need your own car and be able to travel to clients homes within the Chico area and surrounding counties.

Physical Requirements : Conditional upon job offer is successfully passing a medical exam that tests amongst other things the ability to move, visual acuity, and hearing acuity. Physical requirements for this role specifically include :

Benefits

You will be reimbursed for the mileage you travel from the office to clients homes.

You will receive a monthly phone stipend.

Eligible for sick benefits after 90 days.

We offer a 401(k) matching up to 4% after 1 year of employment and 1,000 hours worked.
