Position Summary :

Working under a grant from the Office of Refugee Resettlement (ORR), NYAP's Post Release Services (PRS) Intensive Case Manager Level 3 provides post release services focused on stabilizing the family unit utilizing trauma-informed and intensive case management. The PRS Intensive Case Manager Level 3 will additionally connect with community-based services to address concerns that require long-term care.This position is required to travel to the initial in-home assessment followed by weekly in-person contacts for the first 45 to 60 days, then monthly in-person or virtual contacts for 6 months, depending on the needs of the family.
