The LSW will assist us in maintaining our distinctive level of care by collaborating with residents / patients and their families to develop plans of care, monitor progress, and ensure success.

The ideal candidate will :

• Demonstrate an understanding of the unique audience dynamics within our facility, and will have the ability to communicate effectively and achieve outcomes among varied audiences
• Convey confidence, clarity, and transparency in all communications
• Be able to proactively and positively work with the multidisciplinary team
• Possess high personal motivation, supporting a strong work ethic, flexibility, and integrity
• Possess the ability to inspire trust and confidence, while maintaining a high degree of professionalism and confidentiality

Responsibilities include :

• Collaborate with the multidisciplinary healthcare team
• Interview residents / patients / representatives and assess their current condition, needs, strengths and weaknesses
• Adhere to and follow the Social Work Code of Ethics and Standards of Practice
• Develop and implement resident / patient-centered plans of care
• Monitor and evaluate clients' progress and modify treatment plans accordingly
• Maintain accurate records and report on clients' status
• Refer clients to community services and assist with coordination of needs to ensure successful discharge plans

Qualifications :

• Must hold a current MN license from the Board of Social Work
• Social perceptiveness and empathy
• Ability to build and maintain professional helping relationships
• Ability to relate and communicate with diverse populations and groups
• Resilience, along with the ability to assess situations and problem-solve for therapeutic outcomes

This is a full-time position, working primarily daytime between 8 : 00 a.m. and 5 : 00 p.m. It includes a rotation of every fourth weekend and on-call holidays.
