RN – Home Health & Hospice

Assignment Details

  • Duration : 13 Weeks
  • Shift : Days + Call

    • Hours : Monday–Friday, 8a–5p

  • Evening / Weekend On-Call Rotation
  • Holiday coverage required (24-hr call on weekends / holidays)
  • Shift Length : 9 hours
  • Guaranteed Hours : 40 hrs / week

    • Cancellation Policy :

    Hospital may cancel up to 4 shifts per 13-week assignment (3 shifts if

    Requirements

  • License to submit : Yes
  • Experience : Minimum 1 year Home Health & Hospice experience
  • Certifications : BLS
  • Driver’s License & Auto Insurance : Required
  • Radius Rule :

    • Candidate must live 50+ miles from the facility

  • Candidate must not be full-time, part-time, or PRN with any Common Spirit / CHI / Dignity facility within the last year
  • RTO : Must be provided upfront;

    • Care Setting

  • Unit : Home Health & Hospice RN
  • Setting : Acute Home Health
  • Charting System : Home Care Home Base
  • Patient Population : Adults, geriatrics, and pediatrics within a 60-mile service radius around Dickinson. Care may be provided in homes, nursing homes, and various community settings.

    • Job Responsibilities

  • Perform initial patient and family assessments; complete health histories and physical assessments
  • Reevaluate nursing needs and update nursing diagnosis
  • Initiate and revise plan of care as patient condition changes
  • Develop goals and incorporate therapeutic, preventive, and rehabilitative interventions
  • Educate patients and families per assessment and care plan
  • Administer medications and treatments as ordered
  • Identify discharge planning needs and implement early
  • Serve as Case Manager for assigned caseload when designated
  • Document clinical notes and update primary physician (minimum every 60 days)
  • Communicate patient needs / changes to physicians and obtain necessary orders
  • Coordinate equipment and services for patient care
  • Supervise and evaluate home health aide performance
