Perform initial patient and family assessments; complete health histories and physical assessmentsReevaluate nursing needs and update nursing diagnosisInitiate and revise plan of care as patient condition changesDevelop goals and incorporate therapeutic, preventive, and rehabilitative interventionsEducate patients and families per assessment and care planAdminister medications and treatments as orderedIdentify discharge planning needs and implement earlyServe as Case Manager for assigned caseload when designatedDocument clinical notes and update primary physician (minimum every 60 days)Communicate patient needs / changes to physicians and obtain necessary ordersCoordinate equipment and services for patient careSupervise and evaluate home health aide performanceJob ID - #968658
