Mobile Crisis Case Manager

POSITION SUMMARY :

Provides support services and interventions face-to-face, within the home and community for individuals (children & adults) experiencing mental health or substance use-related crisis.  This individual will need to be prepared to provide immediate, short-term and evidence-based practices to screen, assess, stabilize, and make appropriate referrals, including crisis stabilization units, inpatient hospitalization, and acute detoxification services.  You will collaborate with the Community Support Services and other departments at PSS to support transitions of care for consumers.  The Mobile Crisis Team operates 24 / 7 during the week- days, evenings, and night shifts, and weekend shifts available.
