JOB SUMMARY : The Family Intake Specialist is responsible for coordinating the timely initiation of referrals to the Early Steps Program. The activities carried out by the Intake Specialist are to assist and enable a child to be evaluated for determination of eligibility for IDEA, Part C and the child’s family to receive the rights, procedural safeguards and services that are authorized under Early Steps. The Intake Specialist strives to be supportive, communicative and responsive resources to families as they navigate through the intake / referral process.
ESSENTIAL JOB FUNCTIONS :
- General reception duties- including phone calls, faxes, filing, charting, etc.
- Receive referrals and review to confirm minimal required information.
- Provide families with information about Early Steps
- Obtain general information from family for intake / referral form,
ensure information received is correct : date of birth, spelling of names, address, contact information, brief reason for referral.
Schedule developmental evaluation appointment with familiesAssign service coordinators to (referral or family) and provide notification of clinic appointmentsMaintain timeliness to ensure compliance per policy specificationsMaintain appointment books and provide clinic sheets for areas assignedMaintain patient files and ensure compliance with Early Steps policy and procedures for documentation.Completes other duties as assigned.Preserves the integrity and confidentiality of protected health information of Easterseals clients.Understands, acknowledges, and implements Easterseals privacy policies involving the disclosure of protected health information of Easter Seals clients.Demonstrates a strong commitment to self-development.Maintains a clean, safe, hazard-free work environment.Completes all assignments and responsibilities, is flexible and a team player.Acts in accordance with the Easterseals behavioral expectations, mission, vision and values.EDUCATION and / or EXPERIENCE :
High School DiplomaGeneral Office / Administrative backgroundKNOWLEDGE, SKILLS AND ABILITIES :
Maintain a strong knowledge base and stay current on job related issues and trendsBasic oral and written communication skillsDemonstrate basic computer skillsPositive attitude towards individuals with disabilities and the Easter Seals work environment and philosophy.Willingness to be flexible.Possess the qualities of integrity, tact, and good judgment.PHYSICAL CAPABILITIES :
Able to receive oral communication via phone and in person.Able to stoop, kneel, crouch, and reach preferred.Able to lift and carry children weighing up to 50 lbs preferred.USE OF TOOLS AND EQUIPMENT :
Able to operate copier, fax and computerBasic knowledge of computer programs, such as Word, ExcelWORK ENVIRONMENT :
