MUST BE available for a minimum of 4 days per week.

prefer M-F but will take Mon / Tues / Wed / Thurs if only 4 days

Volume : 20 patients per day

Epic Training is every other week 8-4 : 30p. Epic training can only happen after credentialing and must be completed before the provider can start.
