Job descriptionCoordinate and oversee patient care activities to ensure efficient and effective service delivery. Collaborate with healthcare teams to develop, implement, and evaluate patient care plans. Monitor patient progress and adjust care plans as necessary to meet patient needs. Ensure compliance with healthcare regulations and standards in all patient care activities. Serve as a liaison between patients, families, and healthcare providers to facilitate communication and understanding. Provide guidance and support to nursing staff and other healthcare professionals. Participate in quality improvement initiatives to enhance patient care outcomes. Maintain accurate and up-to-date patient records and documentation. Assist in the development and implementation of policies and procedures related to patient care coordination. Bachelor of Science in Nursing (BSN) from an accredited institution Current Registered Nurse (RN) license in the state of practice Minimum of 3 years of clinical nursing experience Previous experience in a coordinator or supervisory role preferred Experience working in community health or outpatient settings is an advantage Strong leadership and organizational skills Excellent communication and interpersonal abilities Proficiency in electronic health records (EHR) systems Ability to work collaboratively with multidisciplinary teams Strong problem-solving and decision-making skills Commitment to providing high-quality patient care and improving health outcomes Ability to manage multiple tasks and priorities effectively

- The RN Coordinator at Neighborhood Family Practice is responsible for overseeing and coordinating patient care activities to ensure the delivery of high-quality healthcare services. This role involves collaborating with healthcare teams, managing patient care plans, and ensuring compliance with healthcare regulations and standards.
