The Case Manager BHU LVN at Mission Community Hospital plays a critical role in coordinating and managing patient care within the Behavioral Health Unit. This position ensures that patients receive comprehensive, individualized care plans that address their physical, emotional, and social needs. The Case Manager collaborates closely with multidisciplinary teams including physicians, nurses, social workers, and external agencies to facilitate seamless care transitions and optimize patient outcomes.
