Job Summary : As a vital member of our interdisciplinary teams, you will organize, prepare, and deliver evidence-based clinical services throughout the continuum of care while upholding the highest ethical standards of practice.
Key Responsibilities :
- Conduct thorough bio-psychosocial assessments and develop tailored care plans :
- Collaborate with the interdisciplinary team to evaluate care levels and risk factors and design coordinated care and discharge plans.
- Implement specific therapeutic methods and interventions based on personalized care strategies.
- Track patient progress, adjusting care plans to align with evolving patient and family needs.
- Educate patients and families on achieving health goals, covering disease prevention, impact of illness, and adherence to treatment regimens.
- Deliver direct patient care including case management, crisis intervention, counseling, and emotional support for patients facing challenges such as trauma or end-of-life care.
- Facilitate referrals to appropriate care levels and resources that align with individualized care plans.
- Document patient interactions promptly and accurately, ensuring compliance with regulatory standards.
- Engage in team activities including meetings and staff education to support clinical and administrative development.
- Contribute to hospital or unit programming aligned with patient needs and operational strategies.
- Work collaboratively to enhance patient experiences while optimizing resources.
Qualifications :
Master's Degree in Social Work, Counseling, Marriage and Family Therapy, or a related field.Experience working in a healthcare environment.Licensed as LMSW, LPC-A, LMFT-A, or CADC.Valid Driver's License.Proficient in computer skills.Strong problem-solving skills, able to make timely and effective decisions.Ability to work collaboratively with supervision and teams for patient assessment and interventions.Excellent communication abilities, fostering relationships across various stakeholders.Competent in addressing age-specific patient needs.Commitment to safety for patients and the workplace.Proactive in self-improvement and staying updated with training and licensing requirements.Culturally competent, understanding social health needs and addressing disparities in care access and provision.Adept at integrating cultural understanding into practice.Benefits : Hartford HealthCare offers a comprehensive benefits package including :
