Job Summary : As a vital member of our interdisciplinary teams, you will organize, prepare, and deliver evidence-based clinical services throughout the continuum of care while upholding the highest ethical standards of practice.

Key Responsibilities :

• Conduct thorough bio-psychosocial assessments and develop tailored care plans :
• Collaborate with the interdisciplinary team to evaluate care levels and risk factors and design coordinated care and discharge plans.
• Implement specific therapeutic methods and interventions based on personalized care strategies.
• Track patient progress, adjusting care plans to align with evolving patient and family needs.
• Educate patients and families on achieving health goals, covering disease prevention, impact of illness, and adherence to treatment regimens.
• Deliver direct patient care including case management, crisis intervention, counseling, and emotional support for patients facing challenges such as trauma or end-of-life care.
• Facilitate referrals to appropriate care levels and resources that align with individualized care plans.
• Document patient interactions promptly and accurately, ensuring compliance with regulatory standards.
• Engage in team activities including meetings and staff education to support clinical and administrative development.
• Contribute to hospital or unit programming aligned with patient needs and operational strategies.
• Work collaboratively to enhance patient experiences while optimizing resources.
