State Licensed Preventative Care Provider

A company is looking for a Telehealth Preventative Primary Care Advanced Practice Provider (NP / PA).

Key Responsibilities

Conduct primarily telehealth video visits, with occasional in-home visits

Perform comprehensive assessments and provide evidence-based care focused on prevention and chronic condition support

Identify care gaps and coordinate with care team members to develop individualized care plans

Required Qualifications

Active Nurse Practitioner (APRN / NP) or Physician Assistant (PA) license

3+ years of outpatient clinical practice, preferably in primary care or related fields

Active NP / PA license in at least one specified state, with willingness to obtain additional licenses

Active BLS certification at time of hire

Comfort with telehealth delivery and electronic documentation systems
