As a Registered Nurse (RN) at our post-acute care facility, you will play a key role in delivering compassionate, patient-centered nursing care to individuals in both short-term rehabilitation and long-term care settings. You will assess, plan, implement, and evaluate individualized care plans to support recovery, improve health outcomes, and ensure the ongoing well-being of each resident.
Key Responsibilities :
Perform thorough assessments of residents and short-term patients to identify physical, emotional, and psychosocial needs.
Administer medications, IVs, and prescribed treatments accurately and safely.
Collaborate with therapy staff (PT, OT, Speech) to support short-term rehab patients in achieving recovery goals and safe discharge plans.
Provide specialized wound care, especially for post-operative or rehab patients.
Monitor vital signs and health status; identify and respond to changes in condition.
Educate patients and families on health conditions, medication regimens, and post-discharge care.
Work closely with physicians, therapists, social services, and other interdisciplinary team members to ensure continuity of care across SNF and rehab units.
Follow infection prevention protocols and contribute to a clean, safe care environment.
Provide timely and appropriate emergency interventions in accordance with facility policy.
Participate in ongoing performance improvement and compliance initiatives to ensure excellence in care delivery.
