Position : Licensed Practical Nurse / Licensed Vocational Nurse (LPN / LVN)\n\nJob Overview\n\nLicensed Practical Nurse / Licensed Vocational Nurse (LPN / LVN) to provide services at our Community Based Outpatient Clinic in Tupelo, Mississippi.\nJob Summary\n\nThe general duty of the LPN / LVN is to provide a wide variety of clinical and administrative nursing care under the direction of the CBOC Clinic Manager, PACT Provider, and STGi Program Manager.\nEssential Functions\n
- Demonstrate basic knowledge of pharmacology and medication administration techniques per facility protocols and state regulations, as prescribed by PACT Provider.\n
- Works with the PACT team to check-in / check-out patients.\n
- Participates in all staff meetings and PACT huddles.\n
- Observes patients and reports adverse reactions to medications, treatment or physical / mental condition.\n
- Performs cardiopulmonary resuscitation (CPR) and assists during respiratory and cardiac arrest procedures.\n
- Performs a range of nursing procedures, including dressing changes, suture removal, oxygen administration, wound irrigation, catheterization, injection administration and venipunctures.\n
- Prepares patients for laboratory procedures and examinations by providing information and instructions.\n
- Sets up and operates therapeutic and emergency equipment, including monitor, EKG, oxygen, AED, and portable suction.\n
- Maintains universal precautions and infection control practices.\n
- Assists support staff as needed.\n
- Documents all pertinent patient information in medical records and VISTA.\n
- Assists in maintaining the VISTA clinic reminder tracking system current for each enrolled patient.\n
- Documents all pertinent information and completes the VISTA Clinical Reminder Program as outlined by the site-specific VAMC for compliance of the Directors Performance Measures.\n
- Assists in scheduling patients for initial and follow-up appointments in accordance with VA access standards and the patient's medical necessity.\n
- Assures patients are informed regarding their plan of care, including lab and diagnostic test results as needed.\n
- Provides health educational materials and resources to patients and families.\n
- Participates in the ongoing Performance Improvement Program between STGi, its Subcontractor(s), and the VAMC.\n
- Assists in ensuring that all required reports are completed accurately.\n
- Assists the CBOC Clinic Manager and other staff in maintaining the clinic's compliance with all federal, state, local, JCAHO, OSHA, Veteran Affairs, STGI VA Program and Subcontractor safety and operational regulations, directives and standards.\n
- Assists the RN Care Manager in ensuring all patient information on access / wait time documentation is complete and accurate.\n
- Maintains confidentiality of all information and supports patients' privacy, rights, and safety.\n
- Applies basic concepts of customer service techniques, cultural diversity and age-specific characteristics.\n
- Documents patient reminders, clinical reminders, clinical interaction and closes encounters, answering questions and performs other duties as needed to ensure a beneficial visit, online activity and / or training and support of the PACT team.\n
- Makes follow-up visits.\n
- Provides general clerical / administrative support coordination work for the unit.\n
- Performs other work-related duties as assigned.\nRequired Skills\n
- Prefer one to three years of current clinical experience working in a similar environment. Outpatient clinical and / or other hospital experience preferred (Medical-Surgical, ICU, ER, or other hospital experience preferred).\n
- Knowledge of basic methods and procedures and complex methods for coordinating and scheduling clinical care.\n
- Thorough knowledge of the variations in anatomy.\n
- Knowledge and ability to apply the practical nursing principles, procedures and techniques.\n
- Ability to assess patient condition, recognize adverse signs and symptoms, and react swiftly in emergency situations.\n
- Ability to follow orders and works under close supervision, while demonstrating initiative and independence.\n
- Ability to provide, as directed by the provider, individualized and group counseling, guidance, and health instruction with the focus on disease prevention and health maintenance.\n
