Houses of Hope is hiring a full-time Intensive Community Services Case Manager for our Targeted Services Coordination Program (TASC). Case Managers assist with planning and problem-solving to overcome obstacles faced by high utilizers identified within the behavioral health system, with goals of improved individual outcomes and reduced overall costs to the behavioral health system. In contrast to traditional case management approaches, which are often tied to single clinical disciplines or to a single service system, Intensive Community Services Case Managers are intended and empowered to cut across organizational and disciplinary boundaries.
Duties and Responsibilities :
- Maintain caseload of 10 individuals with multiple serious and chronic behavioral health diagnoses (6 hours or 6 contacts face to face per month).
 - Complete an initial assessment to identify resources and needs and coordinate the development of an individualized, goal-oriented service plan for each client.
 - Assist individuals in achieving objectives and maximizing independence and community integration through onsite support, advocacy, modeling, and training in use of personal and community resources.
 - Assist in developing formal and informal community supports and network of family, friends, and professionals.
 - Participate in an on-call rotation to provide 24-hour individualized care.
 - Participate in weekly staffing supervision meetings with collaboration partners.
 - Be an active participant in community meetings and individual-oriented team meetings.
 - Increase job knowledge through involvement with in-service trainings and educational trainings appropriate for the position.
 - Participate in appropriate and prompt record keeping promoting individualized care and community integration.
 - Implement a wrap-around approach to support individuals, family members, and other significant persons in their lives.
 - Administer "flex-funds" per program and state guidelines as applicable for each individual on caseload.
 - Transport clients as needed using personal vehicle.
 
Requirements :
Bachelor's degree in psychology, sociology, or related human services field, or two years direct care experience or training in human resources field.CPR / First Aid required (will provide training at no cost).Proof of valid driver's license and insurance is required.Minimum age of 21Full-time Benefits :
