Wrights Healthcare & Rehabilitation is a trusted provider of skilled nursing and rehabilitation services in Largo, Florida. Our in-house subacute rehabilitation program is designed to support patients in their recovery journey following surgery, injury, or illness. We are currently seeking a dedicated and compassionate Full-Time Physical Therapist to join our dynamic rehab team.

Job Summary :

The Physical Therapist (PT) will be responsible for evaluating, planning, and implementing physical therapy services for residents in our subacute rehab unit. The PT will collaborate closely with interdisciplinary team members to promote patient progress and successful discharge outcomes.

Responsibilities :

• Conduct thorough assessments to identify patients’ physical therapy needs.
• Develop and implement individualized treatment plans focused on functional recovery and mobility.
• Provide direct therapy services in accordance with physician orders and facility protocols.
• Monitor and document patients' progress toward goals and modify plans as necessary.
• Participate in care plan meetings and interdisciplinary team discussions.
• Educate patients, families, and caregivers on therapeutic exercises and aftercare strategies.
• Maintain accurate and timely clinical documentation in compliance with regulatory requirements.
• Support a patient-centered, goal-driven approach to care delivery.
