A compassionate and skilled Hospice Registered Nurse (RN) is needed to provide expert end-of-life care and emotional support to patients in their homes or residential settings within the Lowell service area. This role requires a strong clinical background and a heartfelt dedication to enhancing quality of life for individuals facing terminal illness.

The RN will oversee and coordinate comprehensive hospice care plans, focusing on symptom management, pain control, and patient comfort. Collaboration with an interdisciplinary teamincluding social workers, chaplains, and CNAsis essential to deliver holistic, patient-centered care throughout the hospice experience.

Key Responsibilities :

• Perform thorough nursing assessments and develop individualized hospice care plans
• Provide skilled nursing care focused on comfort and symptom relief
• Administer medications and treatments per physician orders and hospice protocols
• Monitor and report changes in patient conditions promptly to the medical director and team
• Educate patients and families on disease progression and care techniques
• Collaborate with interdisciplinary team members to address patients physical, emotional, and spiritual needs
• Maintain accurate and timely documentation using Home Care Home Base (HCHB)
• Participate in team meetings, case conferences, and quality improvement initiatives
• Ensure compliance with hospice policies, procedures, and regulations

Qualifications & Desired Experience :

Active RN license in good standing within the state

Minimum 1 year recent hospice nursing experience

Proficiency with Home Care Home Base (HCHB) electronic documentation system

Strong clinical assessment skills with knowledge of pain management and end-of-life care

Valid drivers license, reliable transportation, and auto insurance

Ability to work independently in a field-based schedule

Compassionate, emotionally resilient, and excellent communicator

BSN preferred

Experience with Medicare hospice regulations and palliative care advantageous

Contract Details :

Full-time, 13-week contract

Schedule : Monday Friday, 8 : 00 AM 5 : 00 PM, occasional weekends or on-call rotation possible

Caseload : Approximately 45 patient visits daily

Daily travel within assigned service area (mileage reimbursement provided)
