American Traveler is seeking a travel Home Health Physical Therapist for a travel job in Lacey, Washington.

Job Description

American Traveler is seeking a Physical Therapist for a Home Health position requiring at least 1 year of recent skilled home health experience and active WA licensure.

Responsibilities

  • Work in a home health setting serving adult patients
  • Typical day involves 4-6 patient visits, with a standard of 27-30 productivity points based on territory
  • Shifts are 8-hour days, 8 : 00 to 16 : 30, Monday through Friday
  • Weekend work may be required and will be discussed during the interview
  • On-call coverage is not required
  • Charting is completed using Home Care Home Base (HCHB), and proficiency is preferred
  • Documentation tools are provided and must be returned at assignment completion
  • Patients are grouped to minimize driving, but occasional travel within a 100-mile radius may be required
  • Mileage reimbursement provided at IRS standard rate; free parking available
  • Must provide care for adult patients in their homes across a designated territory
  • First-time travelers are welcome to apply
  • Travel pairs are accepted, but each traveler is assigned to their own territory and must have personal transportation
  • Must wear professional scrubs in any color
  • Requested time off must be submitted at the time of application
  • Must be separated from former employment for at least 6 months and approved by HR to return as a traveler
  • Timely documentation and effective communication with the care team are required

    • Requirements

  • Minimum 1 year of recent skilled home health PT experience required within the last 3 years
  • Current BCLS certification required
  • Active Washington PT license required; license required if applicable
  • OASIS experience required, especially for Start of Care
  • Proficiency with Home Care Home Base (HCHB) preferred
  • Active driver's license and personal transportation required
  • Ability to communicate and document in a timely manner
  • Completion of Hospice / Home Health Checklist required
