We are seeking a Nurse Practitioners - FNP, PMHNP to become a part of our team! You will focus on providing high quality patient care as part of a healthcare team.

Position Summary
The Nurse Practitioner (FNP or PMHNP) is responsible for providing advanced clinical care to patients within an integrated behavioral and physical health model. The practitioner will conduct comprehensive assessments, diagnose and treat medical and / or psychiatric conditions, prescribe medications as appropriate, collaborate with the care team, and contribute to individualized treatment plans that promote wellness and recovery.
Essential Duties and Responsibilities
Conduct patient evaluations, including health histories, physical examinations, and mental health assessments.
Diagnose, manage, and treat acute and chronic illnesses or psychiatric disorders within the scope of licensure.
Prescribe and manage medications, reviewing side effects and ensuring safe pharmacological practices.
Collaborate with interdisciplinary teams, including therapists, case managers, and medical assistants, to coordinate holistic care.
Review and sign treatment plans, progress notes, and medical necessity documentation in compliance with regulatory requirements.
Provide education to patients and families regarding diagnoses, treatment plans, and preventive health care.
Participate in case conferences, quality improvement meetings, and care coordination activities.
Maintain accurate, timely, and compliant documentation within the Electronic Health Record (EHR).
Support integration of behavioral health and primary care services to improve patient outcomes.
Adhere to HIPAA, AHCCCS, and ADHS standards for confidentiality and clinical practice.
Specific Role Expectations
For FNPs (Family Nurse Practitioners) :
Focus on primary and preventive care for clients, including chronic disease management and wellness education.
Conduct annual wellness exams, lab reviews, and coordinate care with specialists.
Collaborate with behavioral health providers to address the physical components of integrated care.
For PMHNPs (Psychiatric Mental Health Nurse Practitioners) :
Conduct psychiatric evaluations, medication management, and follow-up appointments.
Develop and monitor psychiatric treatment plans in collaboration with counselors and case managers.
Provide crisis stabilization, risk assessments, and referral coordination when appropriate.
Qualifications
Master’s or Doctorate degree in Nursing from an accredited institution.
Current Arizona RN and APRN license with national certification (FNP or PMHNP).
Current DEA registration (or eligibility to obtain).
Minimum of 1 year of experience in outpatient, integrated care, or behavioral health setting preferred.
CPR / BLS certification required.
Strong interpersonal and communication skills.
Ability to work independently and collaboratively in a team-based environment.
Physical and Work Requirements
Ability to sit or stand for extended periods during patient care or documentation.
Must be able to use computers and EHR systems efficiently.
Occasional travel to outreach or community partnership sites may be required.
