A company is looking for an LPN Health Coach for Care Management.

Key Responsibilities

Provide support for clinical outcome management and healthy lifestyle information to patients

Complete transitions of care calls for moderate risk patients and arrange follow-up services

Assist patients in managing chronic diseases and provide health literacy through instruction and resources

Required Qualifications

Certificate Program Post High School Vocational / Specialized Training Required

1 year of healthcare experience required

Licensed Practical Nurse (LPN) or Multi State License upon hire required

Basic Life Support certification upon hire required

Completion of required education programs and competency testing upon hire required
