POSITION DUTIES  Care Coordination / Continuum of Care / Patient Focus and Care Plans PREFERRED QUALIFICATIONS  RN License / BLS / Acute Care Experience LENGTH OF ASSIGNMENT  13 weeks SHIFT / HOURS PER WEEK  Days 8-5 / 40  START DATE  7 / 22 / 24
