Conduct screening, assessment, and crisis intervention via telehealth and in-personDetermine appropriate level of care and least restrictive setting for clientsProvide transitional support, advocacy, and care coordination until clients begin or resume ongoing treatmentRespond to crisis calls in partnership with local law enforcementDeliver ongoing mental health treatment for clients in intensive case management programsDevelop initial treatment plans and transition clients to long-term servicesMaintain accurate documentation and recordsAbout the Location

Job Requirements
