Setting : A stand-alone skilled facility is a type of specialized healthcare setting that provides a high level of medical and rehabilitative care for patients recovering from a hospital stay. Unlike a nursing home for long-term care, a skilled facility focuses on short-term treatment and therapy, such as physical, occupational, and speech therapy, and medical services like IV therapy and wound care. These facilities are staffed by licensed medical professionals, including registered nurses, and are designed to help patients return home after an illness, injury, or surgery.
The practitioner functions as a Family Medicine / Internal Medicine primary care provider, responsible for admitting, rounding on, and discharging patients within this environment. This role necessitates close collaboration with the administration and active participation in weekly interdisciplinary team (IDT) meetings.
