MLTC Intake Nurse - RN\nLocation : Brooklyn, NY (Hybrid)\nLanguage Skills : Chinese or Spanish a strong plus\nSalary : $95,000 - $105,000 annually\nEmployment Type : Full-Time\n\nA mission-driven Managed Long Term Care (MLTC) program serving the diverse communities of Brooklyn is seeking a Registered Nurse to join its Intake team. This is a hybrid role requiring field-based assessments and in-office collaboration.\n\nThe MLTC Intake RN plays a vital role in evaluating potential new members for program eligibility, educating them and their families on available services, and supporting a smooth transition into long-term care. Bilingual language skills in Chinese or Spanish are highly preferred due to the populations served.\n\nKey Responsibilities : \n

• Conduct in-person assessments using the UAS-NY tool to determine program eligibility and safety for potential members.\n
• Educate prospective members and families about the MLTC program, services, and enrollment process.\n
• Collaborate with the Intake / Outreach and Clinical teams to coordinate timely, appropriate onboarding of new members.\n
• Communicate professionally with referral sources, physicians, caregivers, and community partners.\n
• Participate in outreach activities to support community engagement and referrals.\n\nQualifications : \n
• Active New York State RN license required.\n
• Bachelor's degree in Nursing required.\n
• Minimum 1 year of acute care experience.\n
• Proficiency in UAS-NY assessments required.\n
• Experience in home care and care management preferred.\n
• Strong communication and interpersonal skills; fluency in Spanish or Chinese is highly desirable.\n
