The Physical Therapist (PT) provides skilled therapy services to residents in a skilled nursing facility (SNF) setting. The PT is responsible for evaluating residents, developing individualized treatment plans, implementing interventions, and working collaboratively with the interdisciplinary team to help residents achieve their highest level of physical function, mobility, and independence.
Essential Duties and Responsibilities :
Conduct thorough evaluations to assess residents’ physical abilities, functional limitations, and rehabilitation needs.
Develop and implement individualized treatment plans based on evaluation findings and resident goals.
Provide direct physical therapy treatments including therapeutic exercise, gait training, balance activities, and functional mobility training.
Monitor and document residents’ progress toward established goals and modify plans as needed.
Educate residents, families, and caregivers on home exercise programs, safety precautions, and functional mobility strategies.
Work collaboratively with nursing, occupational therapy, speech therapy, and other team members to coordinate care and ensure continuity.
Participate in care plan meetings and discharge planning as needed.
Maintain accurate, timely, and compliant documentation according to facility and regulatory requirements.
Supervise Physical Therapy Assistants (PTAs) and support staff in accordance with state practice regulations.
Ensure services meet professional standards, facility policies, and state / federal regulatory guidelines.
Qualifications :
Education : Graduate of an accredited Physical Therapy program.
Licensure : Current and valid Physical Therapist license in the State of Nevada.
Certifications : Current CPR / BLS certification.
Experience : Prior experience in a SNF or geriatric setting preferred, but new graduates are welcome to apply.
Skills : Strong communication, time management, documentation, and teamwork skills.
Physical Requirements :
Ability to lift up to 50 pounds and assist residents with mobility and transfers.
Frequent standing, walking, bending, and reaching required.
