Job Title : RN Home Health

Address : Rome, GA

Job Description

Administer skilled nursing care for clients of all ages in their place of residence.
Coordinate care with the interdisciplinary team, patient / family, and referring agency.
Assess home health and / or hospice patients to identify their physical, psychosocial, and environmental needs.
Implement, develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
Educate patients and / or their families regarding care, diet, medications, and home safety.
Supervise and provide clinical direction to staff to ensure quality services.
Monitor assigned cases to ensure compliance with requirements of third party payer.

Critical Information

Shift : M-F until shift is complete; on-call required if requested.

Local travel required in Floyd, Bartow, Gordon, Chattooga, Murray, Pickens, Gilmer, and Catoosa counties.

Education / Licenses Needed

Current state license as a Registered Nurse is required.

Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program is required.

CHPN certification is preferred.

Proof of current CPR certification is required.

1-2 years experience as a Registered Nurse in a clinical care setting is required.

2+ years Home health experience is required.

Benefits :
