Overview

The Cardiologist is a Licensed / Board Certified professional who plays a key role as part of the clinical operations team, providing direct patient care and assessments, primarily in the ambulatory health care center setting. The responsibilities include diagnosing and treating cardiovascular diseases, taking medical histories, conducting physical exams, developing care plans, providing health education, and coordinating referrals. The clinician participates in clinical rounds and conferences in support of ChenMed standards for quality, service, productivity and teamwork.

Essential Job Duties / Responsibilities

  • Provide excellent inpatient and outpatient cardiovascular care, care coordination, echocardiogram reading, population management of market members, and oversight of PCP cardiovascular skills.
  • Lead a cardiovascular care team that may include Care Promoters, Echocardiogram / X-ray Technicians, and mid-level providers dedicated to cardiology depending on scope and member volume.
  • Assist the Medical Director, Cardiology in managing day-to-day clinical and business operations and build positive relationships with providers and stakeholders across ChenMed / JenCare / Dedicated Senior Health centers.
  • Ensure compliance with guidelines, participate in risk and quality management programs, and engage in meetings that maximize clinical value and patient satisfaction.
  • Manage care of chronic cardiovascular diseases for a panel of patients (minimum 5,000) with support from the care team and NPs / PAs as appropriate.
  • Assess patient suitability for surgeries, assist surgeons in strategy for patient management, and clear patients for surgery when indicated.
  • Perform and interpret EKGs, chest X-rays, and echocardiograms; order appropriate blood tests and ancillary services as needed.
  • Limit external cardiovascular care when high-quality internal care is available; prescribe medications in line with patient needs, payer guidelines, and industry standards.
  • Review external cardiology referrals, determine urgency, and coordinate follow-up or consultations as delegated.
  • May oversee cardiology operations for 2-5 centers under supervision of the MD, CVC.
  • Exercise final medical judgment for panel patients; provide patient education on heart disease, wellness, prevention and early detection.
  • Assist with training and hiring of cardiology Care Promoters and NPs / PAs; coach PCP physicians and clinicians in cardiology management.
  • Participate in cardiovascular team meetings, daily huddles, and market / super-huddles; contribute to quality, cost-reduction and strategic initiatives; build relationships with physician stakeholders.
  • Perform other related duties as assigned.

Knowledge, Skills And Abilities

  • Thorough knowledge of pharmacological agents used in patient treatment.
  • Strong clinical skills in managing seniors with complex medical needs; effective communication to build relationships with external organizations and PCP teams.
  • Analytical skills with the ability to manage for results; conflict management and population risk management experience preferred.
  • Professional verbal and written communication; proficient with electronic medical records and office software; strong interpersonal skills.
  • Ability to travel locally, regionally and nationwide up to 15% of the time.
  • English fluency; independent judgment in clinical decisions.
  • Education And Experience Criteria

  • MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar; Board certification in Cardiology required (sub-specialties welcome).
  • Current, unrestricted medical license in the state of employment; current DEA and DPS registration.
  • Minimum of 2 years’ experience as a Cardiologist; experience with interdisciplinary teams in quality improvement or medical leadership preferred.
  • BLS certification from AHA or Red Cross required within first 90 days of employment.
