Position Overview :
The Community Liaison plays a crucial role in connecting our healthcare services with the community. This individual is responsible for driving business development and managing communication effectively. As a leader in the local market for B2B, B2C, and DTC engagement, the Community Liaison represents our organization to referral partners, patients, and caregivers.
Key Responsibilities :
- Business Development & Outreach :
- Identify opportunities for in-services in the healthcare community and coordinate with relevant personnel.
- Build and maintain strong relationships with referral sources, such as hospitals, physician offices, senior living communities, and healthcare providers.
- Seek and pursue new business opportunities to grow our patient census across service lines.
- Represent the organization at community events, health fairs, and professional networking functions.
- Patient & Family Engagement :
- Educate patients, families, caregivers, and legal representatives on the benefits and services of Hospice.
- Guide families through the enrollment process with compassionate and knowledgeable support.
- Communication & Coordination :
- Develop relationships with medical staff offices, hospitals, and nursing homes; provide consultations as necessary regarding hospice and end-of-life information.
- Facilitate seamless communication between referral sources and our clinical teams.
- Ensure timely follow-up on referrals and maintain accurate documentation of outreach activities.
- Market Leadership :
- Provide educational opportunities for physicians, their staff, and hospital social workers / discharge planners.
- Collect and analyze data on referral patterns from all sources, such as physicians and nursing facilities.
- Maintain current knowledge of the community's healthcare market, competition, and policies affecting outreach goals.
- Act as the face of the organization in the local market, promoting brand awareness and trust.
- Monitor market trends and competitor activity to inform strategic planning.
Skills and Experience Required :
Proven experience in healthcare marketing, sales, or community outreach, preferably in home health, hospice, or a related field.Demonstrated ability to meet or exceed performance targets, including referral volume and community engagement metrics.Strong interpersonal and communication skills, allowing effective engagement with diverse audiences.Knowledge of Medicare / Medicaid and additional payer sources is a plus.Understanding of hospice eligibility and levels of care is preferred.Familiarity with customer referral source management systems for activity tracking.Able to work independently and collaboratively in a fast-paced environment.Valid driver's license and reliable transportation are required.Preferred candidates will have a track record of using data to drive decisions, enhance outreach strategies, and fulfill overall business development goals.
