Imagine the Possibilities requires a dedicated and compassionate individual to join our team as a Direct Support Professional I. In this role, you will be instrumental in empowering people to reach their goals and strive for more.

Job Description :

  • Collaborate with supervisors and programmers to develop personalized goals for the individuals served.
  • Provide services tailored to meet the unique needs of each individual, including teaching skills, implementing behavioral strategies, motivating participation, and ensuring safe environments.
  • Document progress accurately and consistently, providing valuable insights into the effectiveness of care plans.
  • Advocate for individuals to have control over their lives, making informed decisions and choices.

Responsibilities :

  • Work closely with HCBS Supervisor and HCBS Programmer to create and implement goal-oriented plans for individuals.
  • Deliver personalized support services based on each individual's unique goals and behavioral care plan, which may include teaching new skills, employing behavioral strategies, encouraging participation, and maintaining a safe environment.
  • Accurately document an individual's progress throughout the duration of the service, highlighting successes and areas for improvement.
  • Assist individuals in creating a safe and supportive living environment by instructing them on best practices for reaching their goals.
  • Act as an advocate for individuals, promoting their autonomy and enabling them to make informed decisions about their lives.
