Job description

Assistant Clinical Service Director – RN

We are seeking an experienced Registered Nurse for an Assistant Clinical Service Director to join a mission-driven long-term care program in Queens, NY!

Schedule

  • Monday–Friday, 8 : 30 AM – 5 : 30 PM

Pay

  • $125,000–$130,000 DOE

    • Daily Responsibilities

  • In conjunction with the Clinical / Center Service Director (CSD), supervises all day-to-day operations of the Interdisciplinary teams and participant in outcomes
  • Coordinate care planning meetings and facilitate resolution of treatment disagreements
  • Balance IDT resources and ensured adequate team coverage
  • Oversee the development and execution of personalized care plansto improve outcomes
  • Ensured timely completion of assessments and care plans in collaboration with centralized teams
  • Implemented in-service training and supported staff development initiatives

    • Required Qualifications

  • Clear and active NY Registered Nursing license
  • 3+ years of administrative experience in a management capacity in CHHA or LTHHCP
  • 3+ years of supervisory experience
