Duties Description Position DescriptionThis RN 1 will provide clinical oversight and coordination of care for individuals receiving services through the Article 16 Clinic, ensuring that medical needs are addressed within an integrated, person-centered treatment plan. This position conducts health assessments, develops nursing care plans, monitors clinical progress, and collaborates with interdisciplinary team members to promote optimal health outcomes. The Nursing Case Manager also serves as a liaison between medical providers, clinic staff, and external healthcare resources to support continuity and quality of care.PRIMARY DUTIES AND RESPONSIBILITIES : Duties will include :

  • Prepare for and participate in telehealth appointments, assisting the provider with clinical assessment including taking vitals when necessary.
  • Maintains and updates charts, records, and other documentation, including tracking necessary lab work, to ensure a high standard of care is provided to patients
  • Ensuring a patient’s plan of care meets their needs and acting as a liaison staff, providers, families or others involved in a patient’s treatment to facilitate modification of the plan as necessary.
  • Will complete appointment preparation for providers including preparing and reviewing documentation that will ensure nurses, providers and anyone else directly involved in the patient’s care have a clear understanding of the goals of the appointment.
  • Reviewing and processing new patient applications to ensure patients are seen in a timely manner relative to their treatment needs and maintaining and clinically triaging waitlists accordingly.
  • Assist with Camp Wilton prep and operation as it relates to medical services at the camp including reviewing medical needs of individuals attending camp, ensuring camp has an adequate supply of medical equipment and supplies to meet the needs of the individuals, and other relevant duties as assigned.
  • Acts as liaison to outside agencies and providers to ensure continuity of care and the necessary follow-up for individuals.
  • Assist as directed in the orientation and training of new staff on duties and operations within the Article 16 Clinic.
  • Participate in various meetings and serving on committees as needed.
  • Attends / completes on-line mandated DDSO training, as well as discipline specific training.
  • Completes special projects as assigned.
  • Incident Investigation and other duties as assigned by Nurse Supervisor IThe Candidate must be able to perform CPR compressions on a supine individual on the floor. The Nurse demonstration of CPR compressions on the floor would ensure nurse ability to perform in worst case scenario, since we use no backboards in OPWDD.SELECTION PROCESS : Selection will be based on interview, and candidates past work history. Minimum Qualifications Must have a license and a current registration as a registered professional nurse in New York State.

Additional Comments This title is part of the New York Hiring for Emergency Limited Placement Statewide Program (NY HELPS). HELPS Program titles may be filled via a non-competitive appointment. This means that you do not need to take an exam to qualify, but you do need to meet the minimum qualifications of the title.At a future date (within one year of permanent appointment), employees hired under NY HELPS are expected to have their permanent non-competitive employment status converted to permanent competitive status. You will not have to take an exam to gain permanent competitive status.

  • Please note that starting salary for this title reflects the increased hiring salary and geographic pay differential. Posted Hiring Salary includes the sum of the statutory hiring rate for PS&T Grade 18 ($66,951), Increased Hiring Salary ($9,196), and Geographic Pay Differential ($20,000). Geographic Pay Differential is added to the job rate ($85,138).
  • If you are currently a New York State employee serving in a permanent competitive qualifying title and eligible for a transfer via Section 70.1 of the Civil Service Law, you may be appointed to this title as a permanent competitive employee.
