Under the supervision of the Nurse Manager / Clinical Coordinator, you will engage directly with patient care on a nursing unit or department, following established policies and procedures.\n\nKey Responsibilities : \n
- Perform ongoing assessments of the patient's physical and psychosocial condition throughout the care continuum.\n
- Develop and document individualized care plans in collaboration with patients, families, and the healthcare team.\n
- Implement and coordinate the care plan while collaborating with the healthcare team.\n
- Evaluate the effectiveness of the care plan, patient responses, and readiness for discharge, making adjustments as necessary.\n
- Assess educational needs and provide tailored teaching opportunities for patients and their families.\n
- Meet performance expectations based on Standards of Professional Behavior.\n
- Perform additional duties as assigned to support the smooth operation of the department / unit.\n\nQualifications : \n\nEducation : Graduate of an accredited nursing program. Proficient in using electronic patient records and internet research for patient care.\n\nExperience : Previous RN experience must not exceed one year.\n\nLicensure / Certification : Valid Connecticut Registered Nurse license. Basic Life Support (BLS) certification required.\n\nSpecial Skills : RNs may be required to work various shifts.
