Expected pay for this position is $31.49 - $37.05 / hour. Actual pay will be determined by experience, skills and internal equity. This is a Salaried position.

Overview

POSITION SUMMARY :

The Adult Pallative Care Counselor provides counseling, critical psycho-social support to palliative care patients through the palliative Care Service consult team. Responsible for the provision of psychological services to patient and their family affected by chronic and / or life-limiting conditions. Conducts appropriate assessment of patient and family mental health needs. Provides consultation to healthcare professionals regarding the impact of psycho-social factors on patients' illness.

Qualifications

• Education : Masters
• Licenses / Certifications : LPC-Licensed Professional Counselor or LSW - Licensed Social Worker
• Experience : 3 years related work experience in social work or counseling
• Required Other skills / knowledge : Excellent interpersonal and communication skills.
