Clinical Team Lead / Clinical Supervisors Responsibilities : (Included, but not limited to)
Teaches and upholds Northern Illinois Hospices standards with a focus on I Am a Teacher, perfect documentation, perfect visits, perfect communication, exceptional customer service and teamwork, and professionalism. acts as a clinical practice expert / resource to staff.Manages the RNs, LPNs, CNAs, Social Workers, Hospice Chaplain / Spiritual Counselors, Aides, and complementary therapists who care for our patients and their family members in their personal residences and in a facility setting.Actively manages teams day-to-day operations, including scheduling, performance, budget, utilization, and quality assessment / improvement processes and methodologies.Develops and coaches staff and creates an excellent team.Provides direct and daily supervision to the team, going out with team members to provide oversight and support to meet Northern Illinois Hospice standards and policy.In tandem with resource nurse, performs initial, annual, and periodic field competency visits with direct reports with focus on infection control, quality, and safety.Adheres to and teaches Joint Commission accreditation standards of practice.Participates in audits to ensure regulatory and agency compliance (e.g., chart audits, skills competency, etc.). Focus made on quality and safety in all work.Manages teams direct care and documentation from admission to discharge to ensure IDG (Interdisciplinary Group) effectiveness. Coordinates teams IDG meetings, demonstrating excellent communication and teaching skill.Requirements of the Clinical Team Lead / Clinical Supervisor :
Graduate from an accredited school of nursing, Associate degree in nursing required, Bachelor of Science in Nursing (BSN) preferred (or obtained within five (5) years).Professional Registered Nurse license issued by the Illinois Board of Nursing.Certified Hospice and Palliative Nurse (CHPN) preferred (or obtained within 2 years of employment).One year of medical / surgical, oncology, or hospice nursing experience.Minimum 2 years of experience as a registered nurse preferred.Active patient contact within past three years preferred.CPR Certification obtained within three months of hire.Ability to work independently, make accurate, and at times, quick judgments.Ability to supervise others appropriately. Ability to respond appropriately to crisis in the community or facility setting.Acceptance of and adaptability to different social, racial, cultural, and religious modes.Meets agency requirements for criminal background check.Salary Description

The expected base salary range for this role is $91,000 - $130,770 . However, final base pay will be carefully determined based on several factors, including education, experience, knowledge, skills, and abilities, as well as internal equity and alignment with market data. Additionally, this role may be eligible for other forms of compensation and benefits.
