Make a difference as a RN Care Manager

As a Full-Time RN Care Manager at VillageCare, you will play a pivotal role in enhancing the health and well-being of our members. Under the guidance of the Care Management Supervisor, you will conduct ongoing assessments of members' health, environmental, and psychosocial statuses, utilizing New York State Assessment tools along with other relevant assessments. Your responsibilities will include developing and coordinating comprehensive care plans through telephonic contact, ensuring that each member receives tailored interventions that address their unique clinical, psychosocial, and financial needs.

You'll oversee the quality and effectiveness of services while facilitating smooth transitions from hospitals or nursing homes. Collaborating closely with the Interdisciplinary Care Team, you will continuously monitor and adjust care plans based on members' evolving needs, aiming to maximize their health and functional capabilities in alignment with local, state, and federal regulations. Your expertise will not only ensure compliance but also empower members to achieve their health goals.

Requirements for this RN Care Manager - Bilingual - Cantonese / Mandarin job

To excel as a Full-Time RN Care Manager at VillageCare, several key skills and qualifications are essential. A valid NYS Registered Nurse License and Registration is required , reflecting your commitment to excellence in patient care. Fluency in Cantonese and / or Mandarin is crucial , enabling effective communication with a diverse member base and fostering trusting relationships. Proficiency in care management software and Microsoft products is necessary for documenting care activities and ensuring compliance with program guidelines. Strong analytical skills will help you assess members' health and psychosocial needs accurately, while your ability to collaborate with the Interdisciplinary Care Team will enhance the overall effectiveness of care planning.
