Medical Assistant

The Medical Assistant is responsible for helping to drive positive outcomes for our patients by providing various administrative duties regarding patient care.

Responsibilities

• Drive self and / or provider to appointments at patient homes
• Obtain vital signs, screen for falls and depression, complete medication reconciliation, review for needed medication refills, review household fall risks (cords, throw rugs, etc.), investigate available food within the home, and serve as scribe for provider during patient appointments
• Observe, record, and report to a provider a patient's condition and reactions to drugs, treatments, and significant incidents
• Conduct patient education as instructed by the provider
• Administer patient care consistent with program protocols and provider orders.
• Facilitate telehealth as needed and deemed appropriate
• Manage PPE supplies and medical bag instruments for both self and assigned provider
• Track patient to be discussed at the interdisciplinary team meeting
• May perform other duties as needed and / or assigned
