Your preventive care efforts will help identify and mitigate health risks, while also coordinating follow-up care to improve health outcomes.

We offer flexible scheduling that allows providers to adjust their time over a 7-day workweek, with no on-call duties, weekends, or holidays required.

Primary Responsibilities :

• Conduct thorough in-home assessments, including evaluations of medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exams.
• Implement evidence-based practice screenings and appropriate point-of-care testing.
• Identify diagnoses for care management and actively manage treatment plans.
• Compile a list of current and past medical conditions based on clinical knowledge and assessment findings.
• Communicate assessment findings to inform the member's PCP of potential care gaps.
• Recognize urgent and emergent situations and intervene as needed.
• Educate members on disease processes, medications, and compliance.

The salary range for this role is between $104,500 and $156,000 annually, based on full-time employment, and may vary based on factors like local labor markets and experience. Additionally, we offer a comprehensive benefits package, inclusive of incentive programs, stock purchase options, and a 401(k) contribution. All benefits are subject to eligibility criteria.
