Optum is looking for a dedicated Nurse Practitioner to become a vital part of our House Calls team. As a clinician-led care organization, we are dedicated to creating a seamless health journey for patients throughout their care continuum.

In this rewarding role, you will be part of the Home and Community Care team, providing essential annual clinical assessments in the comfort of patients' homes. Your preventive care efforts will help identify and mitigate health risks, while also coordinating follow-up care to improve health outcomes.

We offer flexible scheduling that allows providers to adjust their time over a 7-day workweek, with no on-call duties, weekends, or holidays required.

Primary Responsibilities :

  • Conduct thorough in-home assessments, including evaluations of medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exams.
  • Implement evidence-based practice screenings and appropriate point-of-care testing.
  • Identify diagnoses for care management and actively manage treatment plans.
  • Compile a list of current and past medical conditions based on clinical knowledge and assessment findings.
  • Communicate assessment findings to inform the member's PCP of potential care gaps.
  • Recognize urgent and emergent situations and intervene as needed.
  • Educate members on disease processes, medications, and compliance.

Required Qualifications :

  • Active NP license or ability to obtain by start date, with no unresolved disciplinary actions.
  • For NPs : Approval for autonomous practice as mandated, if applicable.
  • Active ANCC or AANP national certification in Family, Geriatrics, Adult, or related fields, or ability to obtain by start date.
  • Ability to meet physical requirements of the job, including transporting a 30-pound bag, navigating various home conditions, and conducting physical assessments.
  • Valid driver's license and reliable transportation for home visits.
  • Preferred Qualifications :

  • Minimum of 1 year of clinical experience in relevant settings.
  • Optional additional licensure in other geographic areas.
  • Experience in gerontology, cardiology, internal medicine, or endocrinology.
  • Background in home health care or home visits.
  • Willingness to travel up to 5% outside your assigned region as needed.
  • Fluency in languages such as Spanish, Cantonese, Korean, Vietnamese, Polish, or others.
  • The salary range for this role is between $104,500 and $156,000 annually, based on full-time employment, and may vary based on factors like local labor markets and experience. Additionally, we offer a comprehensive benefits package, inclusive of incentive programs, stock purchase options, and a 401(k) contribution. All benefits are subject to eligibility criteria.
