Cascade Community Healthcare (CCH) has been providing treatment services to Lewis County and surrounding areas for more than 50 years. Cascade currently has six treatment locations in Lewis County.
We are looking for a Behavioral Healthcare Navigator to join our Adult Services Team! The Behavioral Health Care Navigator is responsible for coordinating care across a spectrum of behavioral health services. This position provides assistance with obtaining Medicaid or Medicare coverage, understanding other insurance coverage, initial screening, and referral services connecting people to needed resources. They may also assist in developing a complete psychosocial evaluation, conducting initial health screenings, and development of a coordination treatment plan. This position is an integral member of clinical services working collaboratively with advocates, clinicians, and peer supports to ensure linkage to relevant services, provides care coordination, case management, and resource coordination. This position supports all individuals enrolled in services or seeking to be enrolled in services, collaborates with all agency programs, as well as other agencies and other community provider

PAY SCALE :
$22.04 - $26.87 Hourly, DOE.
WHAT YOU NEED TO GET THE JOB DONE :
- Bachelors degree in counseling or the social sciences required.
- Washington State Registered Agency Affiliated Counselor (RAAC) credential required, or eligibility to obtain.
- Two years experience is preferred.
- Must be able to pass WA State Patrol background check. A national background check may be required if have not lived in WA state consecutively the last three years.
- Must comply with agency policies and procedures.
- Must have a valid driver's license.
- Must be able to operate a motor vehicle.
- Must be able to gain access to all types of buildings and residences regardless of the structures handicapped accessibility or lack thereof.
- Must maintain current phone number and mailing address on file with the human resources office.
- Excellent verbal and written communication skills.
- Must demonstrate the ability to work as part of a team.
- Must be able to represent the agency in a positive and professional manner.
- Organizational and task management skills.
- Proficient with Microsoft Office Suite or related software.
WHAT YOU'LL DO :
Coordinates, track, and follow up on internal and external referrals.Utilizes aspects of the SBIRT model (Screening, Brief Intervention, and Referralto Treatment), that can include referrals to other community partners.Assists individuals and families with identifying and prioritizing goals.With individuals, formulates time-limited care coordination plans that targetclearly identified care coordination goals with a means of measuring successand program completion.Facilitate health insurance applications and enrollment processes for eligible uninsured individuals.Educate the individual about resources in their home community.Attends and assists with agency events, setup and tear down Cascade tables at community events representing Cascade in a professional mannerMaintains, records and reports in accordance with agency policies and procedures.Participates in supervision, consultation, and training at the direction of the Program Manager.Must be able to complete all paperwork associated with position according to agency policy and procedures.Other duties as assigned by program manager / director.WHAT YOU'LL LOVE ABOUT US :
