What Youll Do

Deliver personalized high-quality care to older adults in homes and senior living settings.

Conduct comprehensive assessments and develop individualized care plans.

Manage chronic conditions with skill and empathy.

Collaborate across clinical disciplines (primary psych palliative neuro) for whole-person care.

Build trusted relationships with residents caregivers and community partners.

Maintain thorough documentation and consistent communication.

What Youll Bring

Active unencumbered NP or PA license in the state you will be practicing

Minimum of 2 years of clinical experience (geriatrics or primary care preferred)

Compassionate patient-centered approach and strong clinical judgment

Valid drivers license and reliable transportation
