Key Responsibilities :

• Perform in-home assessments and develop comprehensive care plans
• Provide skilled nursing care, including post-surgical care and chronic disease management
• Complete timely and accurate OASIS documentation using Homecare Homebase
• Educate patients and families on medication adherence and self-care techniques
• Participate in on-call rotations as needed
