Key Responsibilities :

Collaborate with interdisciplinary teams to develop and implement individualized care plans

Conduct rounds, review documentation, and oversee medication management for accuracy and compliance

Lead new resident admissions, complete essential assessments, and ensure readiness of households

Participate actively in QAPI initiatives and continuous quality improvement projects

Manage incident reports, compliance documentation, and ensure adherence to safety regulations

Required Skills :

Strong knowledge of federal, state, and local healthcare regulations

Proficiency in electronic medical records and healthcare software

Excellent communication skills, tactfulness, and professionalism

Ability to multitask effectively in a fast-paced environment

Strong interpersonal skills to support diverse residents and team members

Commitment to confidentiality and HIPAA compliance

Nice to Have Skills :

Training in memory care or specialized geriatric care

Previous management or supervisory experience in long-term care

Bilingual capabilities are a plus

Education and Experience :

Minimum of two years of clinical experience, preferably in a hospital or long-term care setting

Prior leadership experience and familiarity with quality assurance programs

Current CPR certification
