Overview

Family Practice Associates was founded by Dr. Jeffrey Foxx in 1983 and has grown to one of the largest family practice groups in Central Kentucky. Today FPA is a group of primary care providers who are dedicated to giving family-centered care from birth to later years. FPA has over 20 primary providers including : physicians, nurse practitioners, physician assistants, and a marriage and family therapist. Responsibilities

The Licensed Professional Counselor (LCSW / LMFT / LPCC) would be responsible for providing professional counseling services to patients and families including direct counseling services, crisis intervention, and coordination of community services / resources. Performance of duties may be autonomous and self-directed as part of an interdisciplinary team. Duties include, but are not limited to :

• Conducts comprehensive, culturally sensitive psychosocial assessments, develops care plans, and provides counseling and crisis intervention services to individuals and families.
• Coordinates services with appropriate clinic and community resources.
• Provides information and education about health / mental health issues to individual patients and to groups.
• Documents assessment, intervention, and treatment data in medical record.
• Any other tasks deemed appropriate and assigned by FPA

Occasional stress in balancing multiple demands and in dealing with patients / families experiencing tension.
