Position Summary :

The Physical Therapist (PT) is responsible for evaluating, planning, and implementing rehabilitative services for patients with physical functioning disorders and / or wound care needs. The PT develops individualized plans of care in line with regulatory and clinical standards, utilizing evidence-based practices to support functional improvement, recovery, and quality of life.

Responsibilities :

Screen, evaluate, and rehabilitate patients with physical functioning disorders and / or wound care needs; develop individualized treatment plans.

Administer therapeutic interventions including manual techniques, ambulation, therapeutic exercises, modalities, wound care, and assistive devices.

Monitor and document patient progress; adjust treatment plans as appropriate.

Evaluate treatment outcomes and modify services based on results.

Provide consultation and counseling to patients, families, caregivers, and service providers.

Communicate effectively with supervisors and interdisciplinary team members regarding patient progress and treatment plans.

Prepare discharge summaries and maintain patient documentation per regulatory and company policies.

Accurately record treatment minutes in compliance with clinical and regulatory standards.

Participate in required team meetings, care conferences, and facility trainings.

Supervise and support PT assistants, rehab aides, and / or students.

Ensure adherence to APTA standards, state practice acts, and professional codes of conduct / ethics.

Submission Requirements :

1+ year of PT experience – Required

LTC or Rehab experience – Required

MO State License – Required

New grads – OK (must have completed Level II experience, not just Level I observation)
