Nevada Licensed Care Manager

A company is looking for a Mom & Baby Care Manager.

Key Responsibilities

Engage members and their support systems through assessments and care planning

Facilitate inter-disciplinary care team meetings and coordinate care to achieve member goals

Educate members about treatment options and community resources to support informed decision-making

Required Qualifications

Nursing degree from an accredited program or a Bachelor's degree in a health care field, or equivalent experience

Minimum of three years of experience in nursing, social work, counseling, or related healthcare profession

Maternity / Mother and Baby experience strongly preferred

Three years of Medicaid and / or Medicare managed care experience preferred

Current unrestricted clinical license as a Registered Nurse, Social Worker, or Professional Clinical Counselor required
