STATEMENT OF THE JOB

Benefits Coordinator will work as part of a team of passionate and dedicated professionals providing effective services and advocacy to ensure clients' well-being, safety, and the greatest degree of independence possible. The Benefits Coordinator will regularly report to the Clinic Director and work collaboratively with clinic staff to collect the necessary information and proper documentation for initial and renewal applications. In addition, you will organize and maintain CCNS benefits tracking spreadsheet, as well as enter all work into CCNS's data tracking system including the progress and outcomes of submitted applications.

• Evaluate client needs and eligibility for public and private benefits and other applicable government programs in partnership with clients' case managers.
• Apply for and maintain government and private benefits for clients, such as Medicaid, food stamps, SSI, Access-A-Ride, etc.
• Keep in constant communication with the government agencies and / or facilities where clients reside on the status of applications via email, phone, or in person.
• Conduct follow-up to ensure clients receive the correct amount of monthly benefits in a timely manner and help to resolve cases in which this has not occurred.
• Escort clients, if necessary, to the government offices to complete the application process.
• Assist in verifying accuracy of services entered for billing.
• Act as resource for Clinical Manager and Administrative Office.
