Summary :

The Community Health Worker role assists patients in accessing resources / services, supports disease self-management, and coordinates culturally appropriate care and services.

This position focuses primarily on supporting youth and their families as well as pregnant and postpartum individuals within the community. Responsibilities include connecting clients to treatment services, resources, assisting with insurance enrollment, maintaining accurate records data, conducting outreach to strengthen partnerships, and connecting clients to resources.

Required Minimum Knowledge, Skills, and Abilities (KSAs)

• Education : High school diploma or GED preferred.
• License / Certifications : If driving required for role, Valid Driver's license, clean driving record, and personal vehicle for use required.
• Experience : Two years of experience working in a social service or health care setting or equivalent educational setting preferred.
• Additional Skills / Requirements Required : Must be a resident of the community being served with good knowledge of the resources of their community.
• Additional Skills / Requirements Preferred : Basic computer skills preferred.
