About the Role :

As a Licensed Vocational Nurse (LVN) at Red Oak Health & Rehabilitation Center, you will play a critical role in delivering compassionate and high-quality nursing care to our residents. Your primary focus will be to support the health and well-being of patients by administering medications, monitoring vital signs, and assisting with daily living activities under the supervision of registered nurses and physicians. You will collaborate closely with interdisciplinary teams to develop and implement individualized care plans that promote recovery and comfort. This position requires a strong commitment to patient advocacy, ensuring that each resident's needs and preferences are respected and met with dignity. Ultimately, your work will contribute significantly to enhancing the quality of life for our residents in a supportive and rehabilitative environment.

Minimum Qualifications :

  • Current and valid Licensed Vocational Nurse (LVN) license in the state of Texas.
  • High school diploma or equivalent.
  • Basic Life Support (BLS) certification.
  • Demonstrated experience in a long-term care or rehabilitation setting preferred.
  • Strong understanding of nursing principles, medication administration, and patient care procedures.

Preferred Qualifications :

  • Experience working in a skilled nursing or rehabilitation facility.
  • Additional certifications such as IV therapy or wound care.
  • Familiarity with electronic health record (EHR) systems.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a multidisciplinary team environment.
  • Responsibilities :

  • Provide direct nursing care to residents, including administering medications and treatments as prescribed.
  • Monitor and record patients' vital signs, symptoms, and overall health status, reporting any changes to the healthcare team.
  • Assist residents with activities of daily living such as bathing, dressing, and mobility support.
  • Collaborate with registered nurses, physicians, and other healthcare professionals to develop and update individualized care plans.
  • Maintain accurate and detailed patient records in compliance with healthcare regulations and facility policies.
  • Educate residents and their families on health maintenance and disease prevention as appropriate.
  • Ensure a safe and clean environment by adhering to infection control protocols and safety standards.
  • Skills :

    The required skills such as clinical nursing knowledge and medication administration are essential for providing safe and effective patient care on a daily basis. Strong observational skills enable the LVN to accurately monitor patient conditions and promptly report any concerns to the healthcare team. Communication skills are vital for educating residents and collaborating with colleagues to ensure cohesive care delivery. Preferred skills like proficiency with electronic health records streamline documentation and improve information sharing across the care team. Additionally, interpersonal skills foster a compassionate environment that supports resident dignity and promotes positive health outcomes.
